I would disagree with tarsal sinus syndrome. Looks like a fracture through the anterior process of the calcaneus on the MR images provided.
Response from author:
We discussed the edema in the anterior process, but thought it secondary to sinus tarsitis.
We did not believe there was conclusive evidence for a fracture in the adjacent calcaneous.
One other thing, the exam was done for chronic pain, not the result of an acute incident to rule out fracture
(although a stress fracture without cortical break may look like this).
